Today’s patient care doesn’t end with the hospital stay. Recovery often continues long after the patient leaves the hospital. Once in the home, UMH Home Care becomes a vital link between patient and physician, ensuring a smooth transition from hospital to the home environment.

UMH Home Care Services assumes the role of physician liaison and patient advocate, providing skilled, high-quality care and promoting independence within familiar surroundings seven days a week.

After a referral is received, a registered nurse will visit the patient at home to conduct a physical and environmental assessment to determine his or her needs. Our team of healthcare professionals then prepares an individualized home care plan for that patient, based on the physician’s treatment plan. Thereafter, each time a home care professional visits, care is provided to the patient and an assessment is performed to determine the patient’s progress and ensure continuity of care and services.
