When recuperating from an illness or injury following hospitalization,
you or someone you know may need some extra time to recover and regain
the ability to perform life’s daily tasks. Our Transitional
Care Unit, also known as a sub-acute care facility, is designed to
meet the needs of adult patients who do not require the full resources
of acute hospital care but are not quite ready for discharge. In
effect, transitional care helps bridge the gap between the hospital
and your home or long-term care facility.
Closer to Home
The Transitional Care Unit (TCU) is located on the fifth floor of
the hospital. This short-term skilled-nursing facility provides medically
stable residents with rehabilitation, clinical monitoring and skilled
nursing care. Its convenient community location enables you or a
loved one to be closer to family and friends while on the path to
recovery. Each resident enjoys a private room, while the comfortable
living and dining areas are open to all residents and their visitors.
Comprehensive physical rehabilitation and occupation therapy is available
within this uniquely home-like environment.

Devoted to the art of healing
During your stay, an interdisciplinary team provides ongoing programs
and activities designed to promote your physical, social, mental
and spiritual well-being.
Coordinated by the nursing staff, these specialists-nutritionists;
physical, occupational, speech and respiratory therapists; case manager;
pharmacists; social workers and physicians-work with you and your
family to develop a personal assessment and rehabilitation plan.
Daily, one-on-one therapy focuses on your needs and your goals for
recovery.

Our goal is to help you or someone you love achieve and maintain
the highest possible level of independence while preserving mental,
physical and psychosocial well-being.
On your pathway to recovery, we will help you:

- Promote,
maintain and restore your good health;
- Access skilled nursing and
rehabilitative care.
Admission to the Transitional Care Unit is subject to the policy
and guidelines established by the New Jersey State Department of
Health and Senior Services.
